Cancellation Policy
If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy, on your part, will make it possible to give your appointment to another patient.
Insurance
In order to provide the high quality of attentiveness and ophthalmic care she insists on in the current healthcare climate, it has become necessary for Dr. Katz to become an out-of-network provider. Patients who have out-of-network benefits can submit a receipt to their insurance plan for reimbursement.
